Momentum Healthcare Staffing is seeking a Nurse Practitioner located in Maine for 5-40 hours a week. This position is for in-home assessments as a w2 employee with a flexible schedule with no on call or evenings. Must be willing to commute to Portland, ME. Compensation is $125 Per assessment with a daily per diem and travel reimbursement. If able to work 40 hours a week:- Hotel, Flight, and Rental Car (Rental Car ONLY provided when they are being flown to a location)- $50/day per diem (paid for their work days)- Mileage reimbursement POSITION SUMMARY:The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, Telehealth visits, and skilled nursing facilities.ESSENTIAL JOB FUNCTIONS -Perform screenings including medical history, physical exam, medication reconciliation and education. - Effectively identify, evaluate, and address the concerns of the member, employee or participant. - Administer and order point of care and lab based testing. - Order and interpret diagnostic imaging, laboratory results and provide clinically appropriate interventions, additional tests, or referral as needed. - Refer members to PCP, case management, disease management programs, and/or appropriate Specialist as necessary. - Maintains positive control of all PHI information and compliance with OSHA and State Board of Nursing.
