Assess risk levels and immediate needs, complete necessary documentation, and handle involuntary admission petitions as required by law. Conduct follow-up visits and calls with individuals exhibiting concerning behavior. Develop and implement personalized care plans, and connect clients to community resources and services. Offer ongoing support and advocacy for clients navigating healthcare and social service systems. Coordinate with police, mental health professionals, and community organizations to ensure effective care and services. Attend team meetings to review cases and improve service delivery, maintaining accurate and timely documentation. Assist with community engagement efforts for individuals with mental or behavioral health challenges. Train law enforcement personnel on mental health issues, crisis intervention techniques, and effective communication strategies. Provide educational resources on mental health and available support services, and offer continuous educational opportunities for yourself and others in the role. Other duties as assigned.
