Nurse Practitioner- Heart Failure Program
HCA Houston Healthcare Southeast has been serving the needs of Southeast Harris County for 55 years. It offers 364 licensed beds, a NICU, an Advanced Wound Care Center, and a full complement of hospital services.
HCA Houston Healthcare Southeast is affiliated with HCA Houston Healthcare, part of the HCA Healthcare Gulf Coast Division. The division is a comprehensive network of hospitals, outpatient surgery centers, emergency centers and diagnostic imaging facilities in greater Houston, Corpus Christi and South Texas. Facilities include: 17 hospitals, nine ambulatory care centers, 13 off-campus emergency centers, and a regional transfer center.
What you will do in this role:
- The Heart Failure Nurse Practitioner serves as an advanced practitioner educator, consultant, coordinator ,researcher. program planner and program implementer
- The Heart Failure Nurse Practitioner conducts initial evaluations with focus on medical history, physical assessment, education, disease management ongoing management and disease prevention..
- The Heart Nurse Practitioner works as a change agent, collaborator between heart failure patients, families nurses physicians administration other healthcare services and community providers.
- The Heart Failure Nurse Practitioner is responsible for ensuring that Services are performing in conjunction with the mission and values of the facility and of HCA, including participation in goals and processes derived from HCA-CCMN initiatives .i.e., core measures.
Qualifications
What qualification you will need:
- Current Texas RN License
- National Nurse Practitioner Certification or
- Registered to practice as a Nurse Practitioner in the State of Texas
- American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification
- Preferred experience as an advanced practice nurse in heart failure acute care setting.
