TheCare Assure Navigator manages the daily operations and workflows of the LungNodule program in conjunction with physician(s) and administrative leadership.The focus of this role will be to manage and follow up on lung nodules foundthrough various avenues including but not limited to high-risk lung screenings,incidental findings in the Emergency Room and inpatient admissions. The CareAssure Navigator will coordinate the next level of care for patients with lungnodules and assist patients, clinicians and program stakeholders in thecollection, reporting and presentation of program outcomes.

Qualifications
Required:
- High school diploma / GED
- Vocational / Technical School
- Associates Degree (2 yr program)
- Minimum 2 or more years clinical, healthcare, or related experience.
Licenses,Certifications, & Training:
- Registered Nurse in the State of Florida (preferred)
- Licensed Practical Nurse in the State of Florida (preferred)
- ARRT (R) (CT) (preferred)
- BLS Certification via American Heart Association
Positionsummary:
- Serve as patient advocate from first suspicious finding to diagnosis.

- Streamline care coordination and paperwork by assisting patient(s) with scheduling appointment(s) and exam preparation.
|
- Support providers and assist patients in understanding their treatment options and resources available (e.g. Smoking Cessation and Pulmonary Rehabilitation).
|
- Empower patients to remove barriers with issues of insurance, transportation, child care, financial resources in conjunction with internal and external resources (ex. Social workers, Financial Counselors)
|
- Conduct follow-up conversations as needed with all patients and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.

- Attend patient care planning conference and other meetings as necessary.
|
- Track and document interventions and outcomes.
|
- Drive process improvement.
|
- Conduct outreach to referrers, providers and other medical professionals.
|
- Stay current on the growing evidence and guidelines associated with lung cancer screening and incidental nodule follow-up.

- Ensure service continuity.
