Kindred at Home, and its affiliates, delivers compassionate, high-quality care to patients and clients in their homes or places of residence, including non-medical personal assistance, skilled nursing and rehabilitation and hospice and palliative care. Our caregivers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families.
As a Home Health Admission Registered Nurse, you will:
Respond quickly to requests for patients seeking information and/or admission to home health.
Meet with patients and families and outlines the strengths of Kindred at Home program(s).
Evaluate patients for eligibility for the home health program.
Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
Complete the admission process with patients and care-givers, being sensitive to the need for education, support and guidance.
Assure continuity of quality patient care delivered with appropriate documentation. Maintain up-to-date patient records so that all visits, contacts, problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Ensure all legal documents, clinical documents and admission protocols are followed precisely.
Maintain knowledge of state licensure, federal certification and accreditation requirements.
Complete pain consultations when requested.
Facilitate communication and education of patients, primary provider, families and caregivers to meet patients needs.
Maintain current licensure certifications and meets mandatory continuing education requirements. Also, maintain up-to-date clinical knowledge by attending at least 12 in-services per year.
Attend interdisciplinary team, staff and departmental meetings as required.
Assist with the development of community relationships to further enhance the presence of Kindred at Home programs.
Perform other related duties as assigned or requested.
Graduate of an accredited school of Nursing. Bachelors degree preferred.
Current state license as a Registered Nurse.
Current drivers license. Must have reliable transportation and insurance.
Proof of current CPR certification.
Minimum of two years experience as a Registered Nurse in a clinical care setting, home health and/or hospice preferred.
