POSITION SUMMARY: The Health Connections Care Management Program assists families with ensuring eligible children receive the medical and behavioral health care services they need to promote optimal health by linking, accessing and supporting families through service integration. The Care Coordinator Supervisor provides regular supervision to a team of up to 7 Care Coordinators (CC), individually and as a group. Oversees all Care Coordinators activities and ensure billable requirements are met and documented correctly so payment is received. Provide coverage in Care Coordinators absence. Promotes Health Connections at community events and represent the program throughout Childrens Aid. RESPONSIBILITIES:Oversee Care Coordinators cases and care management activitiesProvide regular supervision to up to seven (7) Care Coordinators; conduct regular and timely evaluation of CCs Manage, review, and monitor maintenance of client records to ensure both internal and external quality assurance standards and timely completion in accordance with Childrens Aid and Health Home PoliciesEnsure billing requirements are met and documented appropriately for reimbursementReview and approve CANS-NY for each of his/her supervisees and complete CANS-NY when neededCompile reports as requestedWork closely with Quality Assurance Manager to ensure quality practice and service delivery, including preparation for auditsParticipate in interdisciplinary team meetings for Care Plan developmentProvide training to new staff and on-going training to existing staffLiaison between the array of Childrens Aid programs and Health ConnectionsRepresent the Health Connections in the community and at other agenciesHelp create linkages to community services for future resource for families as well as source of potential referralsField work as needed (possibility of evenings or weekends)Maintain confidentiality & HIPAA complianceWork as part of the Health Connections team in all areas, focus on meeting established targetsParticipate in Program, Division and Agency wide activities (meetings, events, etc.)Attend trainings as requiredOther duties as assigned SKILLS AND QUALIFICATIONS:Masters degree (LMSW/LCSW, LMHC, or related field) plus 2 years of supervisory experience requiredExperience with Care Management a plusExcellent organizational, time management skills, and attention to detailStrong communication skills verbal and writtenSelf-motivated team playerAbility to multitaskComfortable with field work and public transportationProvide on call coverage as neededComputer literate Outlook, Word, Excel, GSI a plusBilingual Spanish a plus
