Position Overview
The ID/DD Coordinator is responsible and accountable for the coordination and administration of services for clients and their caregivers under the direct supervision of the Location or Executive Director.
Essential Job Functions
• Provide initial evaluation to determine appropriateness and safety of client and environment.
• Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws
• Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis
• Participate in the development of Individual Support Plan
• Maintain patient confidentiality
• Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested
• Maintain effective communication between staff, supports coordinators, and family
• Review progress notes from paraprofessionals to determine appropriate service delivery
• Educate paraprofessionals on individual client ISP expectations
• Perform on site evaluation of employee performance at 90 days and annually
Requirements
• Bachelor’s degree in Psychology or other social science or related field
• Current CPR certification
• Proof of eligibility to work in the United States
• Current completion of child abuse clearance and Fingerprinting

• Minimum of one year of experience with ID/DD preferred

Physical Requirements
• Must be able to speak, write, read and understand English
• Must be able to travel; company does not provide vehicles or transportation
• Frequent lifting, carrying, pushing and pulling of 25 pounds
• Occasional lifting, carrying, pushing and pulling of 50 pounds
• Prolonged walking, standing, bending, kneeling, reaching, twisting
• Must be able to sit and climb stairs
• Must have visual and hearing acuity
• Must have strong sense of smell and touch
Environment
• Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
• Possible exposure to blood, bodily fluids and infectious diseases
