Opportunity Overview:

Uncapped productivity income potential with base
Average 6-8 hours daily rounding
Leadership stipend

Responsibilities:

Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facility
Provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrollees
Work alongside pharmacists on medication management
Complete acute, follow up, advanced care planning, discharge, and wellness visits as necessary
Implement an individualized plan of care for patients' goals
Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
Timely response in chart completions and pharmacy recommendations

Experience as a physician in a skilled nursing facility preferred. However, physicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), internal (IM), emergency (EM), primary, adult, post-acute, home health, hospice, and palliative medicine.

Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $193,000 to $333,00 annually with no cap on productivity income potential.
